Current Therapy 2018 Vol.36 No.9 65治療薬解説897（bTMB陽性19））であることが確認された未治療NSCLCを対象としたアテゾリズマブとプラチナ併用療法を比較する第Ⅲ相試験（B -FAST試験）も進行中である．Ⅶ おわりに本邦でも2つのPD -1阻害薬に加えて，PD -L1阻害薬であるアテゾリズマブが使用可能となった．ニボルマブとアテゾリズマブはともにPD -L1陰性に対しても使用可能であるが，ニボルマブが最適使用推進ガイドラインにより2次治療での使用を制限されているのに対して，アテゾリズマブはSqかつPD -L1陰性の症例に対する使用に注意喚起がなされている以外には使用の制限はなされておらず，今後使用の機会が多くなることが予想される．すでに開始されているプラチナ併用療法後のドセタキセル＋ラムシルマブとアテゾリズマブを比較する第Ⅲ相試験（EMERALD試験）などを通して，将来，適切な患者に適切な治療が提供されるようになることを期待する．一方，PD-（L）1阻害薬と化学療法との併用療法に関する有望なデータが次々に報告され，今後は初回治療からPD -（L）1阻害薬を使用する方向性になることは間違いない．薬剤の組み合わせは多岐にわたり，ニボルマブとイピリムマブ（CTLA4抗体）などのI -O＋I -O併用療法20）も含め，どの組み合わせが最良であるかはまだ答えは出ていない．単剤療法と同様に今後の治療経験や比較試験が，適切な患者に適切な併用療法を提供することに繋がるよう強く期待する．参考文献1）Rittmeyer A, Barlesi F, Waterkamp D, et al；OAK StudyGroup：Atezolizumab versus docetaxel in patients with previouslytreated non-small-cell lung cancer（OAK）：a phase3, open-label, multicentre randomised controlled trial.
