Current Therapy 2018 Vol.36 No.11 87治療薬解説1127面的効果）の存在が注目され始めた10）．この効果のなかでスタチンが心血管イベント発症を抑制する一因としてこの抗炎症作用が挙げられ，ACS発症後30日と4カ月でのhs-CRPの値が高値であるほど2年後の死亡率が高く，スタチンの投与はこのACS発症後30日と4カ月での高感度CRPの値を有意に低下させたという報告がある11）．これはACSにおける炎症反応の重要性とスタチンの抗炎症作用を臨床的に示したものである．しかしながらスタチンはあくまでもコレステロールの低下を目的とした薬剤であり，抗炎症効果は副次的な効果に過ぎない．今回の心血管イベントの抑制は二次予防が目的で，かつ対象患者がhs -CRP高値の者に限定されているが，抗炎症をメインターゲットとした治療が奏効していることには大きな意義がある．また，今回の試験では肺癌の発生も大きく抑制しており，心血管疾患と悪性腫瘍という現代の死因の大多数を占める疾患を一元的に抑制できる可能性を示すカナキヌマブの抗炎症療法は，今後の展開に期待が寄せられる．参考文献1）Libby P：Molecular bases of the acute coronary syndromes.Circulation 91：2844-2850, 19952）Ridker PM, Cushman M, Stampfer MJ, et al：Inflammation,aspirin, and the risk of cardiovascular disease in apparentlyhealthy men.
