Current Therapy 2016 Vol.34 No.9 13急性大動脈解離の病態解明837は手術となり退院時死亡率は22％，一方，閉塞B型のそれはわずかに3％であり，型により大きな差がある．閉塞A型は10％，開存B 型は9％であった（急性大動脈スーパーネットワーク）．? まとめ急性大動脈解離の臨床像に関して病態から治療まで概説した．重要な循環器疾患であるにもかかわらず不明な点は多い．特にその発症機序に関してはほとんどわかっていない．また，急性期からの経口薬のなにが最も予後を改善するのかもエビデンスは不足している．治療の重要なモダリティーとしてのTEVARをB型解離にどのように使っていくのかはこれからの課題である．そのような意味では大動脈解離は古くて新しい疾患であり，今後の研究成果が待たれる分野である．参考文献1）村井達哉：大動脈解離と突然死?東京都監察医務院における1,320剖検例の統計的研究．日法医誌 42：564-577, 19882）Williams DM, Lee DY, Hamilton BH, et al：The dissectedaorta：part Ⅲ.
