12 Current Therapy 2016 Vol.34 No.2116いのも確かだろう．それでは将来展望として今後のガイドラインはどのような方向で進化することが望ましいのであろうか？ 前述したように従来のガイドラインは，①高難易度の手術で，②リスクが低くなく，さらに③心房細動を良性疾患と想定したうえで多くの制限が付いた記述であった．しかし本稿で述べたように今後は，①難易度がそれほど高くなく，②手術リスクも許容範囲で，③ 単に良性とは言えない疾患に対する手術に対するガイドラインに徐々に進化してゆくことが期待される．しかし一方ではやはり度を超えた適応拡大や安易な適応決定に歯止めをかけなくてはいけないことはガイドラインの重要な役目であり，適応の拡大と制限をバランス良く両立してゆくことが重要な課題となるであろう．本稿ではあえて現行のガイドラインを批評する形での記述を行ったが，その真意は実臨床の先生方が心房細動患者の治療方針を考えるうえで，ガイドラインだけに頼ることなく，常識的な判断に基づいて専門施設に御紹介いただきたいとの気持ちから発している．本稿が心房細動患者および実臨床の先生方の一助となれば幸いである．参考文献1）循環器病の診断と治療に関するガイドライン（2010年度合同研究班）：不整脈の非薬物治療ガイドライン（2011年改訂版）．（http://www.j -circ.or.jp/guideline/pdf/JCS2011_okumura_h.pdf）（2015年11月閲覧）2）Brooks AG, Stiles MK, Laborderie J, et al：Outcomes oflong-standing persistent atrial fibrillation ablation：a systematicreview.
