26 Current Therapy 2016 Vol.34 No.10962Ⅳ おわりに現存の膝OAの治療法は，外科的治療法も含めすべて症状改善型治療法であり，疾患の進行を遅延もしくは修飾できる治療法は存在しない．膝OAの究極的な治療法は，関節軟骨の変性と摩耗を抑制する方法であろう．しかし，単純X線ではK/L分類グレード2以上の膝OAを呈するが無症状の1,700万人が，グレード2未満の膝OAを発症していない無症状者に比べ，日常生活動作（activity of daily living：ADL）や歩行能力が劣るとするエビデンスはない．さらに，MRI等の画像診断技術の発展から，K/Lグレード2未満であっても，膝OA性変化により疼痛を伴った患者が多く存在することも明らかとなっている13），14）．したがって当面は，疼痛をはじめとした症状を有する場合が治療対象となり得ることに変わりはないものと考える．しかし，「ロコモによる移動能力維持あるいは回復」の観点からは，疼痛やその他の結果が反映されるADLの維持あるいは回復がより重要となるのではないかと考える．末期膝OAの高齢女性（平均年齢74歳）に対して人工膝関節置換術を行った場合，術前因子のなかで術後6カ月時のADLの影響因子を多変量解析にて検討すると，それは術前の疼痛強度やADLスコアではなく，術前の片脚立脚時間で評価した運動機能評価値であった15）．手術による疼痛改善効果はeffectsize（ES）で評価すると2.8と強力である一方，片脚立脚時間改善効果はESで0.5であった（注：ES 小：0.2，中：0.5，大：0.8以上）．今後病態への理解がさらに進むことで，保存的治療および外科的治療双方において，疼痛のみにとらわれることなく，移動能力をいかに低下させないか，例え移動能力が低下しても速やかにかつ安全にこれを戻すことを考えることも大切になるのではないだろうか．そのためには，治療を行うタイミングと治療法の選択や，それをより安全かつ確実に実施するために必要なことはなにか，という視点からの対策と工夫も重要となるものと考える．参考文献1）Murray CJ, Barber RM, Foreman KJ, et al；GBD 2013DALYs and HALE Collaborators：Global, regional, andnational disability-adjusted life years（DALYs）for 306 diseasesand injuries and healthy life expectancy（HALE）for188 countries, 1990-2013：quantifying the epidemiologicaltransition.
