70 Current Therapy 2014 Vol.32 No.121232た大規模な臨床試験はない．また，アスピリンよりも出血のリスクの少ないクロピドグレルとの併用についての臨床試験も行われていない．現在，CSPS.com（Cilostazol Stroke Prevention Study. Combination）として，脳梗塞再発ハイリスク群を対象としたシロスタゾールを含む抗血小板薬の併用療法の臨床試験がスタートしており，その結果が待たれる．Ⅳ おわりにCHANCE（Clopidogrel in High -Risk Patientswith Acute Nondisabling Cerebrovascular Events）によって脳梗塞の急性期でのアスピリン＋クロピドグレルの抗血小板薬併用療法の有用性が示された8）．しかし慢性期の再発予防に関しては，この組み合わせの併用療法は再発を若干低下させるものの，出血合併症や死亡率の増加が認められ（図2），これまでのところ長期にわたるアスピリン＋クロピドグレル併用療法の有用性は報告されていない．しかし，MATCHでは半数がハイリスクのラクナ梗塞であること，CHARISMAでは脳梗塞の臨床病型がさまざまであり脳梗塞以外の幅広いアテローム血栓症を対象としていること，SPS3ではアスピリンの投与量が通常量よりも多いこと，臨床病型がラクナ梗塞のみであること，など脳梗塞の再発予防に臨床応用するうえでの問題も多い．また，日本人は出血性脳卒中の頻度が高いこと，本邦ではシロスタゾールのように海外では普及していない抗血小板薬もあること，画像診断を駆使した臨床病型を詳細に分析することができることから，病態に則した最も的確な併用療法の有用性が検討される必要がある．参考文献1）Diener HC, Bogousslavsky J, Brass LM, et al；MATCHinvestigators：Aspirin and clopidogrel compared with clopidogrelalone after recent ischaemic stroke or transient ischaemicattack in high-risk patients（MATCH）：randomised,double-blind, placebo-controlled trial.
